Welcome to the John D. Dingell VA Medical Center

We are pleased that you will join us at the Detroit VA for your Clinical Training. This web page has the information to allow you to receive your VA badge and computer codes by the start of your rotation. You must start this process a minimum of eight weeks prior to your rotation date.

Forms to complete:

All forms must be completed in their entirety a minimum of eight weeks before the start of your rotation of your training program.

You will need the free Adobe Acrobat Reader to access these PDF forms.  The Adobe Acrobat reader can be downloaded from Adobe.  https://www.va.gov/viewer.htm  

For Fellows, Residents, Student, and Trainees:

Please complete Computerized Patient Record System (CPRS) training module before your rotation begins.
Please complete VA Trainee Satisfaction Survey after your rotation is complete.

Required Eight weeks prior to start of your rotation: Submit to the department you will be rotating with. Complete blocks 1-17. Sign as an Applicant.

Required Eight weeks prior to start of rotation, complete and submit to the department you will be rotating with. Please fill this form out online because it has drop down fields. If you hand write in a response that is not one of the drop down choices your application could be rejected. Medical students submit form to Corey.Johnson2@va.gov.

On-line training should be completed at least eight weeks prior to the start of the rotation to avoid rescheduling the rotation. To complete the course, go to the Talent Management System (TMS) and follow the Step By Step Instructions to create a TMS account. This training is a yearly requirement. Your VA Location is DET. Please see Point of Contact by Program list below.

Eight Weeks prior to start of rotation, go to room B5310 (red tower 5th floor) to complete the fingerprinting and badge process. Two acceptable forms of identification will be required. See Accepted Forms of Identification word document above. Government issued, not expired. Please contact the department you will be rotating in when fingerprinted. Medical Students contact, Corey Johnson (corey.johnson2@va.gov).

Eight weeks prior to start of rotation complete the Application for Health Professionals (except section four) submit to the department you will be rotating with. Medical students submit form to Corey.Johnson2@va.gov.

Acknowledgement of possible Random Drug Testing.
Sign, date and submit to the department you are rotating with.

Fellows and Residents only

Required Eight weeks prior to start of rotation, print this letter and fill in the specialty service, sign, date, print name, home address, and school address. Do not fill in program start and end date, this will be filled in by the VA coordinator.

Residents Only

Required Eight weeks prior to start of rotation complete the SF61 to include the following blanks: date appointed, department, your name above the oath of office, and your signature as applicant. No notary required.
