Services include — home-based primary care home telehealth care adult day health care community nursing home care skilled and unskilled home care home hospice care home IV therapy Geriatric Research Education and Clinical Center (GRECC) The GRECC integrates models of clinical care, education and research to further the care of geriatric patients with dementia. The GRECC's memory loss clinic is a key component in the continuum of care of patients with Alzheimer's disease.
