How to add a Community Living Center or Domiciliary page
The Veteran-centered purpose
As a Veteran, family member, or caregiver, I want to understand what a CLC or domiciliary is, and learn about what health services it provides, so that I can understand what kind of care I can receive at a CLC or domiciliary.
Content intent
The Community Living Center and domiciliary detail pages are the official Veteran-facing records that describe to Veterans, caregivers, and their families the following information:
-
What a CLC or domiciliary is
-
Who is served and eligibility
-
How to contact the facility and business hours
-
Facility location, parking, and transportation
-
Types of care your facility provides to Veterans
Before you start
-
You must have a CMS account
-
You must create a new detail page
You must be able to answer the following questions:
-
What specific services do you offer Veterans?
-
Are referrals required? (Yes/No)
-
Are walk-ins accepted? (Yes/No)
-
Can the Veteran schedule an appointment online? (Yes/No)
-
Where is the facility, and what are the business hours?
-
What are the visiting hours and visiting policy?
-
If the program is within another facility, which buildings or rooms does the program occupy?
-
Which options are available to Veterans, caregivers, and their families for parking? Is there a cost for parking?
-
How will transportation be provided to Veterans if needed?
-
Are wheelchairs available? (Yes/No) If so, where?
Page structure and guidelines
The purpose of these pages is to provide access to care. We recommend making as few edits to the page template as possible, but in a way that provides Veterans, caregivers, and their families the specific information they need to contact you and benefit from your services.
Even if you wish to share a lot about your program with your audience, editing this template in a concise, clear manner will result in a better user experience and better communication to your audience (less is more).
The rest of this article breaks the page into each section of the web page from top to bottom, and describes how edits should (and should not) be made.

If I’m a Veteran, can I use community living services?
You may be eligible for the VA Coatesville Community Living Center Program if you meet all of the requirements listed below.
All of these must be true:
• You're signed up for VA health care, and
• We conclude that you need community living services to help with your ongoing treatment or personal care, and
• There is space available
We may also consider other factors, like your service-connected disability status or insurance coverage.
Your VA primary health care provider or VA social worker can contact us in 2 ways to start the screening and admissions process:
• They can submit a request online (this is called a consult), or
• They can call our admissions coordinator
Note: As a Veteran, you can also call us directly to start the admissions process. You should be ready to share medical and financial documents when you call.
If you're not currently in the VA health care program, you can apply for benefits now.
